Complete the Client Checklist below prior to your appointment with Flourish Furnishings. This form covers what to expect, procedures, and acknowledgements of the nature of items available.

NOTE: The questions on this application are in sync with the most current form, dated April 7, 2020. However, due to COVID, some of the procedures have been modified and are not yet updated on the paper form available here and downloadable from their website.

Each item must be initialed indicating acceptance and understanding.

This form is in sync with the most current version of the Flourish Furnishing form (dated 04-07-2020). However, some questions are no longer applicable due to COVID safety procedures. Please initial them anyway per Flourish Furnishing participation requirements.

If I cannot make it to the appointment at the scheduled time, I must notify my caseworker at least 24 hours prior to reschedule or there will be a $25 no show fee assessed. *

I am responsible for securing a vehicle large enough to pick up my items. I must arrive / check-in with the vehicle or my appointment will be forfeited and a $25 no show fee assessed. *

NOTE: Moneytalk can work with U-Haul to get donated or discounted use of a vehicle for this purpose. Coordinate with your Moneytalk Case Manager.

It is recommended to bring others to help load and secure items in my vehicle. I understand they will not be allowed on the shopping floor. *

No more than 2 people should accompany the client. NOTE: Due to COVID, all selections will be made through Zoom, clients are not currently allowed inside the building. Staff/Volunteers will load the vehicle for you.

I understand that Flourish Furnishings does not guarantee condition or quality of any specific item when I shop. I cannot hold them liable for any real or perceived issues with the items I receive. *

You are not allowed to select items to be given to someone else for their home.

There is no use of cell phones while on the shopping floor. *

NOTE: Do not make personal calls, text messages, check emails, etc. while on the Zoom shopping call.

Children should not be brought to your appointment. If I bring children to the shopping appointment, I will also bring an adult to care for them during my shopping appointment. Flourish Furnishings does not have childcare facilities and children are not allowed in the shopping area. *

I will be accompanied through the warehouse by a Furnishings Ministry representative for my appointment. The personal shopper will guide me through the process and communicate the guidelines for what is available to me based on availability at the time of shopping. No one else is allowed on the showroom floor. *

NOTE: Current procedure is for all shopping to be completed through a Zoom session where you (the client) will be attending from inside the vehicle in the Flourish Furnishings parking lot.
